  1. Contractors Questionnaire
  2. Work-In-Progress Schedule 
  3. Personal Financial Statement of All Stockholders
  4. Bond Request – Award Letter for Performance/Payment Bond (Complete only for Bid or Performance and Payment Bond request only). 
  5. 3 Years Business Financial Statements either (CPA prepared or In house Quickbooks generated).
  6. 3 Years Business Federal Tax Returns
  7. Current Bank Line of Credits, amount available
  8. Copy of General Liability Insurance Certificate
*Bond Submissions are not binding until all conditions have been met and the bond is issued.
